I hereby authorize Unlimited Network Of Opportunities International Corporation (UNO) to collect my personal information and process the data indicated herein for the purpose of contract tracing effecting control of the COVID-19 transmission, in compliance with the mandate of the Philippine Government and pursuant to the Joint Memorandum Circular No 20-04-1, s. 2020 (“DTI and DOLE supplemental Guidelines on Workplace Prevention and Control of COVID-19”).

UNO requires all its visitors to accomplish the customer/visitor contract tracing form or contracting app provided prior to entry.

Personal Data

Personal data collected in the contact tracing form and app are as follows:

  • Complete Name
  • Temperature
  • Mobile or Phone Number
  • Complete Current Address
  • Email Address
  • Birthdate
  • Date of Visit
  • Time of Visit

UNO collects these personal data when you manually accomplish the UNO’s contact tracing form or electronically use the UNO contract tracing app.

Purpose and Use

UNO collects, process and use your personal data in compliance with Joint Memorandum Circular No. 20-04-A s. 2020 (“DTI and DOLE supplemental Guidelines on Workplace Prevention and Control of COVID-19”) for contact tracing purposes only.

Protection Measures and Risk Management

UNO respect and value your rights as a Data Subject under the RA 10173 or the Data Privacy Act (DPA) of 2012. UNO International Corp. is committed to protecting personal information that it collects, uses and/or processes in accordance with the requirements under the DPA and its implementing rules and regulations (IRR).

For this purpose, UNO implements reasonable and appropriate physical and technical security measures during collection, transmission, storage, and disposal of your personal information.

Personal data provided shall only be shared with the Department of Health (DOH), appropriate agencies following all existing protocols and personnel tasked to process, compile, safekeep and destroy these data. These personnel are duly informed of their obligation to treat these information with greatest confidentiality.

The exchange of these data may be facilitated through email or hard copy.

Personal Data collected by physical forms are protected and stored in locked filing cabinets monitored by CCTV cameras while those via electronic means are stored in a well-maintained database managed by our system expert.

Data Retention Period and Deletion

I understand that my personal information is protected by RA 10173 or the Data Privacy Act (DPA) of 2012 and that all forms collected will be destroyed through shredding and those electronically submitted shall be deleted from the database after thirty (30) days from the date of accomplishment and submission, in compliance with Joint Memorandum Circular No 20-02-A.

Accuracy

The accuracy and correctness of any personal data provided shall be your responsibility as well as the consequences of your failure to disclose your correct personal data to UNO.

Data Subject Rights

UNO commits to uphold the data privacy rights of all its data subjects in compliance with RA 10173 or the Data Privacy Act (DPA) of 2012.

Filing of Complaint

Pursuant to the DPA and its IRR, data subject has the right to file a complaint should there be any violation of his/her data privacy rights.
